*Specialized Healthcare

First and foremost, please recognize that I am a healthcare provider at which I take my chosen profession very seriously.  I am personally committed to the overall well being of each of my patients.  I ask that you respect our time together as much as I do.  Also, realize the time you have taken for yourself is not a pampering service, but a form of continued healthcare that has tremendous beneficial outcomes.  I specialize in massage therapy that offers treatments to reduce pain & tension.  If you are interested in receiving massage only to “pamper” yourself, I can recommend other businesses in the area that provide great service.  It’s not my intention to persuade you to go elsewhere, but in my best interest to make sure you have found a great fit in a massage therapist.       

 

*Payment

Full payment is due at the time of service, unless other arrangements have been made and stated on the financial disclosure.  Cash and checks are acceptable methods of payment in person.  Credit card payments are taken via my website at http://www.annrossmassage.com and must be made within 24 hours of each session.

 

*Rates

Standard Massage Therapy rates are $25 per 15 minute increment of service.  All insurance claims are billed a standard rate of $100 per 1 hour session.  If you are unable to pay for your session at the time of service and will not be using insurance, you will be subjected to the standard rates.  Loyalty courtesy credits are given to those individuals paying at the time of service.  Please see Prices/Specials page for current pricing.

 

*Cancellation/No Show Policy

Appointment times are held for each client individually.  This is your time set aside specifically for you as a reservation.  If you are unable to make your appointment please allow 24 hours notice so other patients can make use of my time.  If you fail to give 24 hours notice or, you fail to show up for your appointment, there will be a $65 charge.  Also, if you are using a gift certificate and you fail to give 24 hours notice or, you do not show up for your appointment, your gift certificate can be used as payment in place of your $65 cancellation fee.  Ultimately, it is my profession & passion to provide the highest level of massage therapy I can to each of my patients.  If you do not give adequate cancellation time or, you do not show up for your appointment, this negatively impacts my patients well being.  I ask that you respect my time as it was your own.

 

*Safety

I have the right to refuse service to anyone at anytime.  This includes the use of drugs, alcohol or any behavior that I feel is inappropriate.  It is very important that I maintain a safe environment for each of my patients at all times.  Please respect the space in which I provide service to each of my patients as much as I do.      


*Insurance
If you have insurance coverage, payment will be insurance co-pays and moneys to be applied towards the deductible.  Your co-payment/coinsurance and/or deductible will be estimated based on the benefit quote from your insurance company.  Ultimately, it is your responsibility to check with your insurance company to see if your plan covers massage therapy, and what your yearly benefit is.  Concerns about coverage for services are between you and your insurance carrier.   I am not an employee of any insurance carrier.  If your yearly insurance benefit for massage therapy has been fulfilled, you will be responsible for all fees incurred for my services, until your benefit period renews.

 

Final payments will be based on the explanation of benefits form that I receive from your insurance company.  Any difference between your original payment and the actual patient responsibility listed on the form will be either refunded, or due at that time.

 

Please note that it is quite possible for another physician to bill a code that falls under massage therapy, resulting in the reduction of your available massages for the calendar year.  It is your responsibility to check with your insurance company to verify available massage therapy benefits for the remainder of the year.

 

A legal guardian must accompany a minor (patient under 18) in order for the minor to receive non-emergency treatment.  Alternative arrangements must be made, in advance of the appointment, if this is not possible.

 

Massage Therapy is an integral part of your rehabilitation program and/or wellness program.  Any feedback or suggestions about my services are always welcomed, encouraged and greatly appreciated.  Thank you in advance for your understanding and compliance with these policies.
